Kristen Black, MSN, RN, CRNP

Kristen Black, CRNP, has been a valued member of the PCA team since 2024, bringing with her years of diverse clinical experience in advanced illness management, home-based care, and orthopedic services.
Prior to joining PCA, Kristen served as a Nurse Practitioner with Wellspan Medical Group’s Wellspan at Home program, where she provided direct medical care across four distinct programs: Advanced Illness Management, Hospital at Home, Primary Care at Home, and Remote Patient Monitoring. This experience gave her a deep understanding of managing complex, chronic, and acute conditions for patients in the comfort of their own homes.
Kristen’s clinical background also includes several years as a Registered Nurse with Wellspan Health’s VNA, where she delivered complex medical care and patient education for a broad range of home-based disease processes. Earlier in her career, she served as a Joint Center Coordinator, RN, with Adventist Healthcare, coordinating the acute surgical care of approximately 850 joint replacement patients annually alongside a multidisciplinary team.
Kristen holds a Master of Science in Nursing from Walden University and an Associate of Science in Nursing from Frederick Community College. She is board-certified as an Adult Gerontological Primary Care Nurse Practitioner and maintains active licensure and prescriptive authority.
Known for her evidence-based approach to care, strong interdisciplinary collaboration, and excellent communication skills, Kristen continues to bring outstanding, patient-centered care to those she serves, and we are proud to have her as part of the PCA team.
